• Through the COR3 Fraud, Waste, & Abuse Hotline toll free hotline number, 1-888-876-7548..
• Through the COR3 Fraud, Waste, & Abuse Hotline website, chat, instant message or email, found through the following link: https://transparencypr.ethicsglobal.com.
Reporting through the COR3 Transparency Portal: https://transparencypr.ethicsglobal.com.
• Click any one of the links mentioned above and select the desired language.
• To the best of your ability and with as much detail as possible, fill out the entire questionnaire. An asterisk (i.e., *) indicates a required field that must be completed prior to submission.
• Once the complaint submission is complete, please review to check the accuracy of your submission.
• Once satisfied, please press “submit” located at the bottom of the webpage.
• You will then receive a report number, which is important to keep for future reference (refer to the monitoring section below).

• Contact the COR3 Fraud, Waste, & Abuse Hotline toll free number listed above to speak to an advisor. First, the advisor will listen, and then ask a series of relevant questions.
• Please have all pertinent information related to the complaint nearby to help facilitate the call.
• The advisor will then give some options to provide any additional supporting documentation you may possess for your report.
• Upon conclusion of the call, the advisor will give you a report number. Please keep this number for your future reference (refer to the monitoring section below).

Using the report number received, you may contact the hotline (through any of the hotline contact means) at any time or check through the Fraud, Waste, & Abuse Hotline website in order to obtain the following information related to the complaint:
• Receive an update on the status of the case/report.
• Contribute any additional information related to the case/report specifics.
• Provide new information regarding any further actions by the parties involved.
• Attach additional supporting documentation to your complaint (via the hotline website).
